Are these Bible true and false questions from the King James Version?

Yes. Names and quoted words follow the King James Version, which is in the public domain. So Isaiah is Esaias in the New Testament, and the reason says so.

Do the statements ask what a church teaches?

No. Each one is about what the text says: who, where, how many, who said it. None turns on doctrine or on one church's teaching.
